Notice to Our Patients
August 25, 2009
Our practice is growing rapidly and our medical staff can provide you with quality one on one attention to make sure you are continuing to utilize proper medications for your illness as well as preventive care measures.
Starting November 1, 2009 we will no longer fill prescriptions without an appointment. We are also requesting that you bring all your prescription bottles with you for your appointment. This will help us better monitor the medication you are taking and make any necessary adjustments. Prescriptions will be filled in three to six month increments depending on your condition. For example, our diabetic or pain management patients require a three month check up; prescriptions will be written in three month increments at the time of visit. Our hypertensive or thyroid disorder patients require a six month check ups, your prescriptions will be filled in six month increments.
We will be offering home visits provided by a certified physician assistant.
Home visits for Medicare covered patients who have difficulties traveling to the office will be covered without additional charge. We will offer same services for our concierge services members without an additional charge.
We will be offering immunization vaccination program, flu & pneumonia vaccine to all patients and the public, without an appointment during office hours, and at Saturday clinic starting October 2009. We are not filing with insurance companies except for Medicare. The vaccine costs will be announced closer to vaccination start date.
You may choose to pay directly $25.00 (vaccine cost) at the time of service, or we will file with your insurance provider if you choose to do so.
You will be responsible for your balance accordingly to your insurance policy.
